Personal Care Coordinator (RN or LPN)
Position Summary
Directs nursing service activities to meet the needs of the residents in the Personal Care facility.
Qualifications
Gerontology experience preferred.
Graduate of an approved school of nursing and licensed in the Commonwealth of Pennsylvania.
Continuing education as required to retain license and stay abreast of changes in the profession.
Essential Responsibilities
Monitors all nursing staff to ensure quality resident care.
Maintains a positive attitude and sets a professional example.
Performs rounds to ensure proper care is being provided.
Assesses resident improvement or decline.
Maintains adequate staffing, redirects coverages for Personal Care
Ensures resident care plans are followed.
Monitors that employees answer call bells in a timely manner and conducts call-bell audits.
Advises Personal Care Home Administrator when counseling or discipline is needed due to deviations from practice or policies.
Participates in training, retraining and mentoring staff to correct any inappropriate behaviors or procedures.
Assists the Personal Care Home Administrator by promptly following up with all new hires, including paperwork.
Accurately assesses staff performance and gives input for evaluations.
Functions in conjunction with the Personal Care Home Administrator by following up on problem/issues identified.
Reviews residents’ condition changes with nurses/aides and assists with resident assessments.
Ensures physicians, families and administration are notified of changes whenever necessary.
Documents and reports any resident or family complaints.
Follows-up as necessary to maintain good communications.
Appropriately handles emergency situations.
Makes rounds with physicians and ensures orders are obtained as needed and in conformance with established standards.
Transcribes physician orders when necessary and ensures that two staff members have noted them for accuracy.
Obtains/provides reports to incoming/outgoing staff, reports all information needed for follow-up, signs the 24-hour report form.
Participates in monthly care plan ZP meetings.
Ensures the communication from the care plan meeting is updated on the care plan documents and pertinent staff is aware of changes.
Monitors dining room and ensures the staff are knowledgeable of diets.
Ensures Admission and Transfer process is followed and documented.
Obtains physician orders for all new admissions/readmissions.
Coordinates a smooth admission process with staff and residents.
Assures Assessments are completed.
Body audits must be done within 2 hours of admission and signed by two licensed nurses.
Performs direct nursing services such as: administering medications and treatments; providing personal care and ADL’s; assessment and evaluation; assistance with diagnostic and therapeutic procedures.
Secures prescribed medications and disposes of medications per facility policy.
Review medication records for accuracy.
Recognizes and reports resident problems which will require referral to other community agencies.
Non-Essential Responsibilities
Follows up on recommendation by the Dietician.
Ensures weekly/monthly weights are done and accurate.
Other related duties and responsibilities that may become necessary as directed by Supervisor.
