Using a collaborative approach, advances, influences, and implements forward thinking clinical practices.
Providing general medical direction and leadership for all aspects of medical care – including coaching, guiding, advising, and educating the clinical teams.
Assures the most appropriate, effective utilization of medical care to ensure the delivery of high-quality participant services.
Provides clinical care services to participants as indicated.
DUTIES AND RESPONSIBILITIES: Leadership/Administrative Assists Medical Director in the overall accountability and responsibility for the medical care and of the participants.
Provides clinical direction to the Primary Care (PC) team to ensure high quality appropriate utilization of care is provided to participants.
Leads the primary team culture and practices, consistent with the mission, values, principles, and philosophies.
Supports, adheres to, and ensures the PC team follows the LIFE-NWPA regulations, philosophies, goals, policies, and processes.
Establishes and ensures quality standards for patient care.
Provides medical leadership under the direction of the Medical Director in the planning and development of care and services in accordance with the LIFE-NWPA model regulations and guidelines.
Provides expertise on medical issues to LIFE-NWPA leaders, CEO, VP- Clinical Care, and Board of Directors on medical matters in collaboration with the Medical Director In collaboration with the Director of quality and VP- Clinical Care, assumes accountability and responsibility for quality improvement at LIFE-NWPA .
Oversees and ensures appropriate medical documentation and record-keeping.
Represents the company with external individuals and community groups.
Participates in developing marketing and outreach strategies Represents the organization with regard to medical issues at other health facilities Participates on LIFE-NWPA Ethics Committee Partners and ensures the implementation of the QAPI program.
In partnership with Nursing, oversees Infection Control Program, including the tracking and analysis of infection data Oversees the LIFE-NWPA pharmacy program and the relationship with the contract pharmacy and consulting pharmacist.
Oversees the submission and assures the quality of diagnostic data submitted to CMS for risk-adjustment.
Maintains appropriate records and participates in data collection efforts.
Attends & participates in staff town halls.
Participates in Leadership meetings.
Represents LIFE-NWPA at the NPA Primary Care Committee and other bodies of the National PACE Association Assists the Medical Director in assuring collaboration of the primary care team members with other disciplines to advance the LIFE care model.
Participant Care: When indicated, acts as the primary care physician for LIFE-NWPA participants in community and institutional settings Ensures PC provides routine, episodic, and urgent care as needed.
Ensures PC performs initial, semiannual, and periodic assessments when indicated.
Leads/manages advanced practice practitioners (APPs) and primary care physicians.
Ensures PC collaborates with the interdisciplinary team, to develop care plans for LIFE-NWPA participants.
Oversees medical care provided by covering physicians Initiates, develops, and maintains relationships with consulting physicians Attends interdisciplinary team meetings as indicated.
Maintains appropriate medical records and related documentation.
Leads improved & innovative ways to deliver high quality care.
Teaching Leads and ensures continued medical education.
Leads & oversees medical trainees working at centers Provides clinical education to staff.
Research Reviews, advises, and supports research projects at the direction of the Medical Director.
Other Stays up-to-date on evidenced base care for frail older adults Provides other reasonably related jobs as assigned MINIMUM BASIC KNOWLEDGE: Experienced Medical leadership and direction with physicians and APPs.
Experience working in managed care setting.
Current knowledge of the physical, mental and social needs of frail older adults is required.
The knowledge, ability, and experience in working with, and advocating for the elderly and their families.
Experience working with a collaborative interdisciplinary team.
Comfort working in multicultural health care environment EXPERIENCE AND QUALIFICATIONS: 3 to 5 years of Medical Leadership experience overseeing physicians and APPs preferred.
Physician holding MD degree and board certified in Internal Medicine.
Prefer Board Certification in Geriatric Medicine.
Prefer 2 – 5 years’ experience in Geriatric Medicine.
The ability to lead and work in an interdisciplinary setting.
Strong EQ, Relationship building, organizational and communication skills.
Ability to manage changing priorities in response to program needs.
Knowledge of cultural factors/disparities of care affecting all ethnic groups served by LIFE-NWPA .
Must meet a standardized set of CMS approved competencies, established by LIFE-NWPA for the position, before working independently.
Must be legally authorized to practice in the state of Pennsylvania.
Licensed physician in the State of Pennsylvania.
Experience or advanced degree in geriatrics is required.
Current state license, DEA registration and staff privileges at LIFE-NWPA and contracted hospitals are required.
Must have 1 year of experience with a frail or elderly population.
(Will only act within the scope of their authority to practice).
SUPERVISORY RESPONSIBILITIES: LIFE-NWPA Primary Care Staff – adviser to all clinical leaders.
Authorized to access full health information.
